BPI has released an update of it's popular Central Office Reliability Report and Database service. 

CO Reliability Reports include a complete reporting of every incident of switch downtime from 2000 to 2008. The service also includes reports that compare the performance of every major switch manufacturer and model, normalized by systems in service and (New this year) lines in service.
